Step 1: Navigate to the Virginia Department of Health’s Covid-19 Vaccination Record Request Portal .
This will bring you to a simple form that requests your Name, Date of Birth, and Zip Code.
Step 2: Enter the requested information and hit “Search”.
Step 3: Successful entry of the requested information will bring you to a new page that will further verify your identity by sending an SMS text message or making a voice call to the phone number that is associated with your vaccination record. The text/phone call will contain a 6-digit verification code. If you do not have access to the phone number that is associated with your vaccination record, please call 877-VAX-IN-VA (877-829-4682) Monday through Friday 8 a.m. to 6 p.m.
Choose your preferred method of contact and hit the respective button to initiate the message.
Step 4: Once you have received your 6-digit verification code, enter it into the box and hit the “Verify Code” button.
Step 5: Successful completion of the previous steps will present a new page containing your COVID-19 Vaccination Record.
What if the COVID-19 Vaccination Record with QR Code is not found?
Vaccine providers in Virginia submit vaccination records to the Virginia Immunization Information System (VIIS), but they may have provided information that’s incomplete, out of date, or incorrect. So, it’s likely VIIS has your record, but not the correct information.
To find the record, you should try re-entering your information without a ZIP code. If your record still isn’t found, you can contact the pharmacy/doctor/nurse who administered your COVID-19 vaccinations and ask them to update your vaccination record. You can also call 877-VAX-IN-VA (877-829-4682) for help; assistance is available Monday through Friday from 8 a.m. to 6 p.m. ET.
Vaccinations administered outside Virginia to Virginia residents may not be in VIIS. Doses administered directly by federal agencies such as the Department of Defense or Department of Veterans Affairs are not reported to VIIS.
